Re20 vs CellREDM: Is There A Difference?

Extracellular Matrix (ECM) skin boosters are the newest injectable categories in aesthetics medicine. Derived from donor skin tissue, human acellular dermal matrix (hADM) sheets have been used in surgery for decades for applications such as hernia repair, breast reconstructive surgery and skin grafting in those with severe burns. In end 2024, with the launch of Re20 in South Korea, hADM made its foray into the aesthetic sphere, in the form of freeze-dried micronised injectables. Demand was so overwhelming that other new entrants quickly followed with the launch of their hADM brands, to ride on the rapid industry boom.

Like many new treatment categories, competition is zeroed in between two main players in the market. ECM skin boosters are now at that stage — The OG Re20 and the newer CellREDM are the two names most patients in this category are choosing between. Given there is currently no extensive studies and data on ECM injectables as of 2026 due to its novelty, most of that choice is currently being made on marketing claims rather than data. As a practitioner, I wanted to dig deeper so patients have something more solid to base that decision on.

Manufacturer Track Record

When it comes to tissue engineered products, the processing steps to ensure that a product is pathogen-free is critical. This makes the manufacturer’s track record an important consideration. The manufacturer of Re20, L&C Bio, has maintained a clean regulatory record across its product lines to date. CellREDM’s manufacturer, Hans Biomed, on the other hand, has had product recalls by Korea’s Ministry of Food and Drug Safety over failure to disclose raw materials used in the manufacturing process. In 2023, there was also a letter issued by the FDA citing significant deviations from regulations for human cells, tissues, and cellular and tissue-based products. While neither case involved CellREDM specifically, they speak to the manufacturer’s quality systems as a whole.

Purity and Net Yield

On paper, CellREDM lists 160mg of hADM per syringe while Re20 is listed as 150mg. However, labelled content and net ECM content aren’t the same thing. hADM requires stringent decellularization processing to remove donor lipids and antigenic components as presence of these residues may trigger an inflammatory or immune response.

Thus, a better indicator is the net ECM purity which takes into account the presence of residual lipid and surfactant in the product. Independent test reports commissioned by L&C Bio shows Re20’s net ECM purity at 99.5%. A 99.5% purity is not something every ECM skin booster can boast and Re20’s high purity profile means not only a high net ECM yield of 149mg, but also an extremely low risk of adverse reactions or side effects.

Particle Size

Lastly, you may have seen clinics claim that CellREDM has a 25% smaller particle size (75 µm) than Re20 (100 µm), which allows CellREDM to spread more evenly and offer higher treatment comfort. This comparison is based on misleading information. When Re20 first launched, the manufacturer stated its particle size as “under 100 µm” — a ceiling, not the actual measurement. This general statement however, led to claims of Re20 being “100 µm” circulating online. In actual, Re20’s particle size is also at 75 µm (for its main whole-face product) and 50 µm (for its eye product). There is thus no actual gap in comfort or spread due to particle size. What actually makes a difference would be the injector’s skill and technique. It’s worth correcting the claim on particle size because it’s one of the more commonly repeated claims that has been widely circulating.

My Take on ECM Skin Boosters

ECM skin boosters as a category are promising, but not all products in the category are backed by the same level of manufacturing rigor or clinical evidence due to the newness of this category. Based on currently available data, Re2O has the stronger evidence base of the two and is, as of 2026, the only ECM skin booster backed by a peer reviewed clinical study.

Overall, this is still a young developing field, and I look forward to more studies and published data that would help practitioners and patients make better, informed choices instead of relying solely on the brands’ own marketing claims.
